Study Abroad Forms
Program participants and interns must complete the following required forms, provide copies of documents, and submit them to Lori Lamb by April 21 , 2017.

- Spain : Seattle University's Education Abroad online Registration.
Complete this online waiver form as required by the University. Follow the step-by-step instructions for ease of use. A copy will go to the law school.

- Medical Report
The Seattle University Student Health Center is located in Bellarmine Hall. The Center will perform a travel health examination for this purpose for a $5.00 fee. Please (206) 296-6300 to make an appointment. When making your appointment, please tell them you will need a travel consultation. Remember to bring the form. You are welcome to seek your own physician's assistance to complete this form.
Immunization Certificate, if applicable
Seattle University Student Health Center offers some immunizations for a fee. Public health centers around King County can provide the necessary immunizations should you need them. You are strongly advised to visit the U.S. Center for Disease Control and Prevention (CDC) Web site before visiting a doctor or clinic.
Guest Form, if applicable
Guests of program participants are required to complete this form as well as provide the same copies of documents.

All students participating in Seattle University School of Law foreign study programs are required to submit this information in conjunction with the insurance plan.
If you are participating in Spain summer study-abroad program, you are automatically covered. The premium fee is separate from the program fee.
If you plan to extend coverage after the study-abroad programs, or if you are participating in international internships, you are required to purchase this insurance plan. You can purchase this plan through Seattle University at the following rates: $20.28 (1-2 weeks); $28.98 (3 weeks); $38.64 (4 weeks), or $48.30 (5 weeks). If longer than 5 weeks, please add $9.66 for each additional week.
The travel medical insurance covers medical (up to $500,000) and global travel assistance insurance in emergency cases such as medical evacuation, family bedside, and repatriation of remains. These are features often not included in regular insurance.See the insurance FAQ for details.
If you are a U.S. citizen, you are encouraged to register with the U.S. Department of State. "Travel registration is a free service provided by the U.S. Government to U.S. citizens who are traveling to, or living in, a foreign country. Registration allows you to record information about your upcoming trip abroad that the Department of State can use to assist you in case of an emergency."
