Summer Abroad Forms
Program participants and interns must complete the following required forms, provide copies of documents and submit them to Junsen Ohno by Friday, April 15 , 2011.

Medical Report
The Seattle University Student Health Center is located in Bellarmine Hall. The Center will perform a travel health examination for this purpose for a $5.00 charge. Please call to set up an appointment at 206.296.6300 and mention that you will need a travel consultation. Remember to bring the form. -
Immunization Certificate, if applicable
Seattle University Student Health Center offers some immunizations for a fee. Public health centers around King County can provide the necessary immunizations should you need them. You are strongly advised to visit the U.S. Center for Disease Control and Prevention (CDC) Web site before visiting a doctor or clinic. -
Guest Form, if applicable
All guests of program participants are required to complete the same forms as the participants as well as provide the same copies of documents. -

Travel medical insurance is included in the program cost for the S. Africa program. If you are participating in other summer-abroad programs or internships, you are required to purchase travel medical coverage. You can purchase this plan through Seattle University for $9/week. If your medical insurance carrier provides this coverage, you need to make a copy of your coverage and submit it to our office for our record. In addition, if you are a U.S. citizen, you are encouraged to register with the U.S. Department of State. "Travel registration is a free service provided by the U.S. Government to U.S. citizens who are traveling to, or living in, a foreign country. Registration allows you to record information about your upcoming trip abroad that the Department of State can use to assist you in case of an emergency."
