Summer Abroad Forms

Program participants and interns must complete the following required forms, provide copies of documents  and submit them to Junsen Ohno by Friday, April 15 , 2011.

Travel medical insurance is included in the program cost for the S. Africa program.  If you are participating in other summer-abroad programs or internships, you are required to purchase travel medical coverage.  You can purchase this  plan through Seattle University for $9/week.  If your medical insurance carrier provides this coverage, you need to make a copy of your coverage and submit it to our office for our record.  Contact Junsen Ohno, International Programs Administrator by phone at 206.398.4283 or via e-mail at ohnoj@seattleu.edu to arrange coverage and for assistance.

In addition, if you are a U.S. citizen, you are encouraged to register with the U.S. Department of State.  "Travel registration is a free service provided by the U.S. Government to U.S. citizens who are traveling to, or living in, a foreign country. Registration allows you to record information about your upcoming trip abroad that the Department of State can use to assist you in case of an emergency."
